Age Well SG (ASG) is a national programme led by the Ministry of Health (MOH) to support seniors to age well in their homes and their communities. Through a comprehensive, long-term transformation across housing, transport, active ageing, and care services, ASG aims to anchor ageing within the community.
As part of the ASG programme, the Active Ageing Centre (AAC) service model will evolve into the new service model, AAC 2.0 – an integrated social and health support model for all seniors. AAC2.0 requires AACs to automate and integrate their processes with the MOH's national IT systems, enabling seamless care delivery and data sharing with MOH, Agency for Integrated Care (AIC) and service providers in the community care sector.
An IT system serving AACs must meet ASG programme-specific requirements under the ASG Certification Framework.
