About Age Well SG
(ASG) Programme

Age Well SG (ASG) is a national programme led by the Ministry of Health (MOH) to support seniors to age well in their homes and their communities. Through a comprehensive, long-term transformation across housing, transport, active ageing, and care services, ASG aims to anchor ageing within the community.

As part of the ASG programme, the Active Ageing Centre (AAC) service model will evolve into the new service model, AAC 2.0 – an integrated social and health support model for all seniors. AAC2.0 requires AACs to automate and integrate their processes with the MOH's national IT systems, enabling seamless care delivery and data sharing with MOH, Agency for Integrated Care (AIC) and service providers in the community care sector.

An IT system serving AACs must meet ASG programme-specific requirements under the ASG Certification Framework.

ASG Certification Framework (Effective Mar 2026)

The ASG Certification Framework includes:

  • Business Requirements;
  • Integration Requirements;
  • Mandatory Standards
    • Obtain the Cyber/Data Security certification; and
    • Adhere to the Data Portability practices

The framework will be updated progressively to incorporate additional features as requirements evolve.

Business Requirements

Equipped with the necessary system capabilities to support AACs’ ABC+2S* service model, including:

  1. AAC Client Management
  2. AAC Reports
  3. Referral Management

*Active Ageing Programmes, Befriending and Buddying Services, Care Referral Services, Social Connector and Community Screening.

Integration Requirements

Integration with web services to automate data sharing with consistent data standards between AACs and MOH / AIC / service providers in the community care sector, including:

  1. SmartCMS web services
  2. HSAR web services
  3. BRIGHT web services

    Your IT system will be required to undergo the ASG certification process. To be certified as an ASG-compliant IT system, your IT system must meet the mandatory standards and ASG programme-specific requirements under the ASG Certification Framework.   • What are the benefits to my company after successfully being certified as ASG-compliant IT system under the programme?

    The following are the potential benefits for companies enrolled in the ASG programme:

    • Enable your IT system to better serve your AAC clients, by providing them an integrated means to perform the necessary system interfaces for regular and effective data sharing, and to meet the new service and reporting requirements.
    • Your IT system will be certified as an ASG-compliant IT system upon successfully meeting all requirements under the ASG Certification Framework and accorded with the ASG programme badge. AACs will be regulated to adopt an ASG-compliant IT system to support their participation in ASG by operating the AAC2.0 service model.
  • Will there be funding or fees associated with onboarding my IT system to the ASG programme?

    There is no funding or any form of reimbursement provided to IT partners for work undertaken for ASG programme. There is no intent to impose subscription fees for onboarding to SmartCMS or utilizing the respective web services at this juncture.


    AACs can apply for the AAC IT grant, which is a one-time funding support to help defray the cost of adopting an AAC IT system or upgrading their existing IT system with capabilities to meet the AAC 2.0 service model.
